Dental RCM

A dental billing team that speaks CDT, not CPT

Dental revenue leaks differently: benefits expire at the annual maximum, crowns need pre-determinations, and a missing narrative stalls payment for weeks. Our dental RCM section covers the whole cycle for dental practices — billing, credentialing, insurance verification, and AR follow-up — inside Dentrix, Eaglesoft, and Open Dental.

  • Dental billing — CDT coding, ADA forms, attachments done right the first time
  • Dental insurance verification — maximums, frequencies, and waiting periods checked before treatment
  • Dental credentialing & AR — PPO enrollment maintained, dental denials appealed weekly

How it works

One claim, followed through all ten steps

Most revenue leaks happen in the handoffs. We own the whole path a claim travels, so nothing is left sitting in a queue nobody watches.

01

Provider Enrollment & Credentialing

Payer enrollment and credentialing completed up front, so providers are approved and reimbursed without delays.

02

Registration & Insurance Verification

Eligibility, benefits, and authorizations verified before the visit to prevent rejections and payment surprises.

03

Accurate Medical Coding

Certified coders assign precise ICD-10, CPT, and HCPCS codes — compliant, and built to maximize legitimate reimbursement.

04

Charge Entry & Claim Scrubbing

Charges entered accurately and scrubbed against payer rules before submission, catching errors while they're free to fix.

05

Clean Claim Submission

Timely electronic submission to every payer, with confirmation tracked until each claim is accepted.

06

Payment Posting & Reconciliation

Payments, ERAs, and EOBs posted accurately, with underpayments and discrepancies flagged the same week.

07

Denial Management & Appeals

Denied claims corrected at the root cause and appealed with documentation — recovering revenue others write off.

08

Accounts Receivable Follow-Up

Dedicated AR specialists work every aging bucket consistently, shrinking days-in-AR and steadying cash flow.

09

Patient Billing & Support

Clear patient statements and a real person to call — better patient collections without burning goodwill.

10

Reporting & RCM Insight

Transparent weekly reporting on collections, denials, and AR — so you always know what your revenue cycle is doing.

Why practices switch

Built for small and mid-sized practices — not scaled-down enterprise software

  • Your systems stay yoursYou keep your EHR and clearinghouse. We log in, learn your workflows, and run the back office inside them — no migration project, no retraining your staff.
  • A named team, not a ticket queueThe same billers, coders, and AR specialists work your account every week. When you call 917-540-0303, you reach someone who knows your payers and your problem claims by name.
  • Nothing ages in silenceDenials and 90+ day balances get worked every week, with a written status you can read in two minutes. If a claim is stuck, you'll know why — and what we're doing about it.
  • HIPAA discipline, in writingSigned BAA before we touch a single record, role-based access, and secure connections into your systems. Your patient data is handled like the liability it is.

Complete revenue cycle management, owned end to end

Hand us the whole cycle — credentialing, eligibility, coding, billing, posting, denials, and AR — and get one accountable team, one weekly report, and cash flow you can plan around.

A billing specialist reviewing claims and accounts receivable across multiple monitors
What's included

Every handoff in the cycle, covered

Revenue leaks live in the handoffs — between the front desk and the coder, the coder and the biller, the denial and the appeal. With complete RCM, there are no handoffs to outside your team: we are the team, and every step below is ours to answer for.

  • Credentialing & enrollment maintained for every provider
  • Eligibility & authorizations verified before every visit
  • Coding, charge entry & scrubbing done daily
  • Payment posting with underpayment flagging
  • Denials & AR worked every single week
  • Patient statements & support handled professionally
  • Weekly reporting on collections, denials, and days in AR
Why it works

What changes when one team owns the cycle

Faster, steadier payments

Claims go out clean and daily, denials get appealed within days, and AR is worked on a schedule — so collections stop swinging month to month.

Numbers you can read in two minutes

One weekly report: what was billed, what was collected, what's denied and why, and what's aging. No portals to dig through, no surprises at month-end.

Your staff goes back to patients

Front desk stops chasing payers, providers stop getting billing questions, and your office manager stops being a part-time AR department.

Medical billing & claims, submitted clean and daily

Charge entry, scrubbing, electronic submission, payment posting, and patient statements — the billing engine of your practice, run every day instead of whenever someone finds time.

A physician reviewing patient accounts and billing documents at his desk
What's included

The daily discipline billing actually needs

Most billing problems aren't knowledge problems — they're consistency problems. Charges sit for days, rejections sit for weeks. Our billing service is built around a daily rhythm your claims can set their watch by.

  • Same-day charge entry from your closed encounters
  • Claim scrubbing against payer rules before submission
  • Electronic submission with acceptance tracked per claim
  • Rejection handling within one business day
  • ERA/EOB posting & reconciliation against your bank deposits
  • Patient statements that are clear enough to actually get paid
  • Secondary & tertiary claims filed, not forgotten
Why it works

Clean claims are a process, not luck

First-pass clean claims

Every claim is scrubbed for demographics, coding edits, and payer-specific rules before it leaves — the cheapest denial is the one that never happens.

Nothing waits for month-end

Charges, rejections, and posting happen daily. Problems surface while they're still cheap to fix — not in a month-end report nobody reads.

Posting that catches underpayments

Payments are posted against your contracted rates, so a payer paying below contract gets flagged the same week — not discovered a year later.

Medical coding that pays in full — and holds up in an audit

Certified coders assign precise ICD-10, CPT, and HCPCS codes with the right modifiers, checked against medical-necessity rules before the claim ever leaves your office.

A clinician reviewing documentation and codes on a laptop
What's included

Coded from the chart, not from habit

Undercoding leaves money on the table quietly; overcoding creates audit exposure loudly. Our coders work from the documentation itself, query providers when the note doesn't support the code, and keep your coding compliant and complete.

  • ICD-10 specificity — coded to the highest supported level
  • CPT & HCPCS selection matched to the documented service
  • Modifier accuracy — 25, 59, XS and specialty-specific modifiers
  • LCD/NCD & bundling checks before submission
  • Provider queries when documentation is ambiguous
  • Chart-to-code audits on request, with written findings
Why it matters

Coding is where reimbursement is decided

Fewer coding denials

Medical-necessity and bundling edits are checked before submission, so coding-related denials drop instead of bouncing back for rework.

Audit-ready documentation trail

Every code traces back to the note that supports it. If a payer audits, your defense is the documentation — already in order.

Feedback that improves notes

Recurring documentation gaps get reported back to your providers in plain language, so the same query doesn't repeat every week.

Credentialing & enrollment, done before it costs you claims

Payer enrollment and credentialing for every provider on your roster — applications filed, followed up, and tracked, with revalidation deadlines watched so nobody silently falls out of network.

Healthcare professionals in a meeting reviewing provider enrollment
What's included

Every payer, every provider, every deadline

A lapsed credential doesn't announce itself — it shows up as a stack of denials weeks later. We keep a live register of every provider's status with every payer, and we start revalidations before they're due.

  • Initial payer enrollment for commercial payers, Medicare & Medicaid
  • CAQH profile setup, attestation & ongoing maintenance
  • NPI, NPPES & PECOS records kept current
  • Application follow-up with payers until approval lands
  • Revalidation & recredentialing deadline tracking
  • New-provider onboarding sequenced so billing starts on day one
  • Demographic updates — address, tax ID, group changes filed with payers
Why it matters

The most expensive denial is "provider not enrolled"

New providers bill sooner

Enrollment starts the day a provider signs, with payer timelines managed actively — not discovered after their first month of claims denies.

No silent lapses

Every revalidation date lives on a tracked calendar with alerts well ahead of the deadline — lapses become impossible to miss.

One register you can ask for anytime

A single status sheet: every provider, every payer, every effective date. Call 917-540-0303 and we read it to you — no digging.

Eligibility & benefits verification, before the patient sits down

Coverage, deductibles, copays, and prior authorizations confirmed 24–48 hours before the appointment — so the visit happens with the money questions already answered.

A verification specialist on a headset call confirming a patient's insurance benefits
What's included

Verified by a person, not just a portal

Electronic eligibility checks miss things — terminated policies, coordination-of-benefits traps, services carved out to another payer. Our team verifies electronically and by phone where it matters, then documents the result in your EHR before the visit.

  • Full VOB 24–48 hours before every appointment
  • Coverage status, deductible & out-of-pocket remaining
  • Copay & coinsurance captured for point-of-service collection
  • Prior authorizations requested, tracked, and confirmed
  • Referral requirements flagged before the visit
  • Patient cost estimates for larger services
  • Same-day add-ons verified before the patient is seen
Why it matters

The cheapest denial is the one prevented at scheduling

Eligibility denials all but disappear

"Coverage terminated" and "not eligible on date of service" are among the most common — and most preventable — denial reasons in small practices.

Collect at the visit, not by mail

Knowing the copay and deductible status up front lets your front desk collect at check-in — money that rarely arrives once it becomes a statement.

Authorizations never block care

Auth requests go out with the appointment booking and are tracked to confirmation, so procedures aren't delayed or written off for a missing auth.

Denial management & AR follow-up that shrinks old balances

Denied and underpaid claims corrected at the root cause and appealed with documentation — and every aging AR bucket worked weekly until the old balances come down.

An AR specialist working aging claims and denial queues across multiple monitors
What's included

Fix the cause, then appeal the claim

Resubmitting a denied claim unchanged just earns a second denial. We categorize every denial by root cause, fix the process that produced it, and appeal with the documentation the payer actually needs — so the same denial stops recurring.

  • Denial categorization by root cause, payer, and provider
  • Corrected claims & appeals with supporting documentation
  • Underpayment recovery against contracted rates
  • 30/60/90+ AR buckets worked every week, oldest first where it counts
  • Payer calls & status checks until claims resolve
  • Timely-filing protection — deadlines tracked so nothing expires unworked
  • Write-off review — nothing written off without a documented reason
Why it works

AR doesn't age because claims are bad — it ages because nobody owns it

A weekly rhythm, not a quarterly panic

Every bucket gets touched every week. Balances can't quietly cross the timely-filing cliff while someone is "getting to it."

Root causes reported, not just results

Your weekly report names the top denial causes and what changed upstream to stop them — eligibility, coding, credentialing, or documentation.

Old AR cleanup projects

Sitting on a 120+ day backlog? We take it as a defined cleanup project with a recovery report — separate from your ongoing billing.

Related services

Dental claims done the dental way

Dental claims fail for dental reasons: a missing narrative on a crown, X-rays never attached, a frequency limit nobody checked. Our dental billers live in those details — it's the only billing they do.

  • CDT coding accuracy across hygiene, restorative, perio, ortho & oral surgery
  • ADA claim forms submitted electronically, daily
  • Narratives & attachments — X-rays, perio charts, intraoral photos sent with the claim
  • Pre-authorizations & pre-determinations for major work
  • EOB posting with PPO fee-schedule & write-off checks
  • Secondary dental claims filed after primary pays
  • Patient statements in plain language patients understand
The dental RCM section

Four services, one dental revenue cycle

Like the medical side, dental works best as a complete cycle. Take billing on its own, or combine it with the other three dental services.

Attachments that don't stall payment

Claims needing X-rays, perio charting, or narratives are identified before submission and sent complete — no 30-day "waiting for documentation" limbo.

Write-offs checked against your fee schedule

Every PPO adjustment is verified against your contracted fees. Incorrect write-offs — a quiet, common leak — get caught and corrected.

Daily submission, weekly reporting

Claims go out the day treatment is completed, and you get a weekly summary of what's billed, paid, denied, and aging — in Dentrix, Eaglesoft, or Open Dental terms.

Dental practices: your audit speaks CDT too

The free audit reviews your dental claims specifically — denial reasons, attachment stalls, write-off accuracy, and AR aging — in a written report.

Dental credentialing that keeps you in network

PPO and dental plan enrollment for dentists and specialists — applications filed and followed up, fee schedules on file, and recredentialing dates tracked so participation never lapses unnoticed.

A dentist treating a patient in a modern dental operatory
What's included

Every plan, every dentist, every renewal

Dental credentialing has its own maze — plan-by-plan applications, umbrella networks, leasing arrangements. We map which networks you're actually in (including leased ones), file what's missing, and keep the register current.

  • PPO & dental plan enrollment — Delta Dental, Cigna, Aetna, MetLife, Guardian & more
  • Network participation audit — including leased-network surprises
  • CAQH & NPI records maintained for every dentist
  • Fee schedules collected, loaded & verified in your practice software
  • Recredentialing deadlines tracked with advance alerts
  • New associate onboarding sequenced with their start date
  • Medicaid & Medicare Advantage dental enrollment where applicable
Why it matters

Out-of-network surprises hurt twice

No lapsed participation

A dentist who quietly drops out of a PPO keeps treating patients whose claims then deny or pay at out-of-network rates. Tracked deadlines prevent it.

Fee schedules you can trust

Your contracted fees are loaded and verified, so treatment plans quote real numbers and write-offs can be audited line by line.

New dentists bill from week one

Enrollment starts when the associate signs — not when their first claims bounce. Timelines are managed payer by payer until approval.

Dental insurance verification before treatment begins

Annual maximums, frequency limits, waiting periods, and downgrades checked before the appointment — so treatment plans are quoted accurately and claims don't come back denied.

A dentist treating a patient whose benefits were verified before the appointment
What's checked

A full dental breakdown, not a yes/no eligibility ping

Dental plans hide the details that decide payment: remaining maximum, last FMX date, missing-tooth clauses. We request the complete breakdown and document it in Dentrix, Eaglesoft, or Open Dental before the patient is seated.

  • Annual maximum — used vs. remaining
  • Frequency limits — exams, cleanings, fluoride, X-rays, replacement rules
  • Waiting periods for basic & major services
  • Coverage percentages by category, incl. downgrades (posterior composites, amalgam substitution)
  • Missing-tooth & pre-existing clauses flagged for implants & crowns
  • Orthodontic coverage & lifetime ortho maximums
  • History — last cleaning, exam, FMX/pano dates from the payer
Why it matters

Accurate estimates mean patients say yes — and pay

Fewer frequency & maximum denials

The two most common dental denial reasons are knowable before treatment. Verification turns them from denials into scheduling decisions.

Treatment estimates that hold up

When the estimate matches the EOB, patient trust goes up and statement disputes go down — case acceptance improves with it.

Front desk time returned

Verification calls are long hold times for your staff. We absorb them and hand back a documented breakdown in your system.

Dental AR follow-up & denial appeals that recover old balances

Aging dental claims worked bucket by bucket every week — denials appealed with the right attachments, narratives rewritten, and insurance AR reported until it's collected or explained.

A specialist working aging dental claims and denial queues across multiple monitors
What's included

Every aging claim gets a next step

Dental AR usually isn't uncollectible — it's unstuck claims: a narrative never sent, an X-ray attached to the wrong claim, a pre-determination treated as a denial. We work each claim to a resolution and report the reason for every balance that remains.

  • Insurance AR aging worked weekly — 30/60/90+ buckets
  • Denial appeals with corrected narratives & attachments
  • Claim status calls to payers until resolution
  • Stale pre-determinations converted or resubmitted correctly
  • Incorrect write-offs identified and reversed where wrong
  • Patient portion follow-up after insurance resolves
  • Weekly AR report — recovered, in progress, and why
Why it works

Dental AR shrinks when someone owns it weekly

Cleanup projects for old backlogs

A defined project for your 90+ day insurance AR: every claim reviewed, recoverable ones appealed, dead ones documented — with a final recovery report.

Appeals written for dentistry

Crown and perio appeals need clinical narratives and the right attachments. Our dental team writes them in the payer's language, with the evidence attached.

Causes fed back upstream

Recurring denial patterns — a code, a payer, a missing attachment habit — get reported so verification and billing stop producing them.

About Vista Medical Billing — the back office behind growing practices

Vista Medical Billing exists for one kind of customer: the small and mid-sized US practice that's too big to bill from the front desk and too small to be a big billing company's priority.

Healthcare professionals meeting around a table with a laptop
Who we are

A named team, not a ticket queue

Every Vista client works with the same billers, coders, and AR specialists week after week — people who learn your payers, your providers, and your problem claims by name. When you call 917-540-0303, you reach that team, not a call-center script.

We built Vista around how small practices actually lose money: not in one big failure, but in a hundred small ones — an eligibility check skipped on a busy Monday, a denial nobody appealed, a credential that lapsed quietly. Our whole operation is a set of weekly rhythms designed to catch the small ones.

  • Your systems stay yoursWe work inside your EHR, practice-management, and dental software. Your data never moves to our systems.
  • Priced on collectionsBilling and RCM are a percentage of what we collect. Credentialing and virtual assistants are flat-fee. No setup fees, no per-claim surprises.
  • HIPAA discipline, in writingSigned BAA before any access, role-based permissions, secure connections only. Your patient data is handled like the liability it is.
How onboarding works

Live in 10–15 business days, with no gap in submissions

01

Free audit & proposal

We review your last 90 days of claims and credentialing status, then quote in writing — percentage of collections for billing/RCM, flat fees where they apply.

02

Access & credential checks

BAA signed, secure access set up in your systems, and every provider's enrollment status verified before we touch a claim.

03

Parallel run

For the first cycle we run alongside your current process, reconciling daily — so nothing is missed and no timely-filing deadline is at risk.

04

Full cutover & weekly rhythm

We own the cycle end to end: daily submissions, weekly AR and denial work, and a weekly report you can read in two minutes.

Talk to the team before you decide anything

One call: we'll ask about your payer mix, claim volume, and where it hurts — and give you a straight answer on whether we'd help.

Questions every practice asks us first

Straight answers on pricing, systems, timelines, and privacy. Anything else — call 917-540-0303 and ask us directly.

Billing and RCM are priced as a percentage of what we actually collect for you — if you don't get paid, neither do we. Credentialing and virtual assistants are flat-fee. Your exact rate comes out of the free audit, once we've seen your payer mix and claim volume; there are no setup fees and no per-claim surprises.

No. We work inside your existing systems — Epic, athenahealth, eClinicalWorks, Tebra, AdvancedMD, Dentrix, Eaglesoft, Open Dental and most others. Your staff keeps charting the way they chart today; we handle everything that happens after the encounter is closed.

Most practices are fully transitioned in 10–15 business days. We start with access and credential checks, run your claims in parallel with your current process for the first cycle, then cut over completely — no gap in submissions, no missed timely-filing deadlines.

We sign a Business Associate Agreement before any access, connect only through secure, audited channels, and limit every team member to the minimum access their role requires. Your data stays in your systems — we don't export patient records to ours.

We review your last 90 days of claims: denial rate and top denial reasons, days in AR, underpayment patterns, and credentialing gaps. You get a written report with the revenue we believe is recoverable — whether or not you hire us. It costs nothing and commits you to nothing.

Yes — small and mid-sized practices are exactly who we built Vista for. Solo providers and two-doctor offices feel a denied claim the same month it happens; you get the same named team, weekly reporting, and weekly AR follow-up as our larger clients.

Yes — dental is a dedicated section of our service, with its own team. We cover dental billing (CDT coding, ADA forms, attachments), dental insurance verification, dental credentialing, and dental AR follow-up, working inside Dentrix, Eaglesoft, and Open Dental.

Yes. Many practices start with a single pain point — an AR cleanup, credentialing for a new provider, or verification for the hygiene schedule — and expand once they've seen the weekly reporting. Every service on this site can be taken on its own.
